Why There Are So Many Types

Blood pressure is regulated by several different mechanisms in the body — the heart's pumping force, the volume of fluid in the bloodstream, the tension in blood vessel walls, and signals from the nervous system and kidneys. Different classes of blood pressure medications target different parts of this system.

This is why two people with the same diagnosis might be on completely different medications. Your doctor chooses based on your specific health profile, other conditions you have, and how your body responds.

The Main Classes of Blood Pressure Medications

ACE Inhibitors

Examples: lisinopril, enalapril, ramipril, benazepril

ACE inhibitors block an enzyme called angiotensin-converting enzyme, which the body uses to produce a hormone that narrows blood vessels. By blocking this enzyme, ACE inhibitors allow blood vessels to relax and widen, reducing blood pressure.

They are often a first-choice medication for people with high blood pressure, especially those who also have diabetes or kidney disease, as they provide additional protection for the kidneys.

Common side effects: A dry, persistent cough is the most well-known side effect, affecting about 10–15% of people. If this becomes bothersome, an ARB (see below) is often substituted.

ARBs (Angiotensin Receptor Blockers)

Examples: losartan, valsartan, olmesartan, irbesartan

ARBs work on the same system as ACE inhibitors but at a different point — they block the receptor that the angiotensin hormone binds to, rather than blocking its production. The result is similar: blood vessels relax and blood pressure falls.

ARBs are often prescribed for people who cannot tolerate the cough caused by ACE inhibitors. They are also protective for the kidneys and are frequently used in people with diabetes.

Beta Blockers

Examples: metoprolol, atenolol, carvedilol, bisoprolol

Beta blockers reduce blood pressure by slowing the heart rate and reducing the force of the heart's contractions. They block the effects of adrenaline on the heart.

They are particularly useful for people who have both high blood pressure and heart disease, heart failure, or a history of heart attack. They are also used for certain heart rhythm problems and anxiety.

Common side effects: Fatigue, cold hands and feet, and sometimes mild depression. They should not be stopped suddenly — always taper under a doctor's guidance.

Calcium Channel Blockers

Examples: amlodipine, nifedipine, diltiazem, verapamil

Calcium channel blockers prevent calcium from entering the muscle cells of the heart and blood vessel walls. Since calcium is needed for muscle contraction, blocking it causes blood vessels to relax and the heart to beat with less force.

They are effective for many people and are often used in older adults and people of African descent, who may respond less well to ACE inhibitors.

Common side effects: Ankle swelling, flushing, and headache are the most common. Grapefruit and grapefruit juice interact significantly with many calcium channel blockers and should be avoided.

Diuretics (Water Pills)

Examples: hydrochlorothiazide, chlorthalidone, furosemide, spironolactone

Diuretics reduce blood pressure by helping the kidneys remove excess sodium and water from the body, reducing the volume of fluid in the bloodstream. Less fluid means less pressure on the vessel walls.

Thiazide diuretics (like hydrochlorothiazide) are among the oldest and most widely used blood pressure medications and are often included in combination pills.

Common side effects: Increased urination (especially when first starting), low potassium levels (which can cause muscle cramps), and increased sensitivity to sunlight.

Combination Medications

Many people with high blood pressure take two or more medications from different classes. Pharmaceutical manufacturers often combine two drugs into a single pill to simplify the regimen. If your medication has a long name with a slash in it (like amlodipine/benazepril), it is a combination product.

Tips for Taking Blood Pressure Medications

Take them consistently. Blood pressure medications work best when taken at the same time every day. Missing doses allows blood pressure to rise, which can increase the risk of cardiovascular events.

Do not stop without talking to your doctor. Even if you feel fine — or especially if you feel fine — do not stop taking blood pressure medication without medical guidance. High blood pressure usually has no symptoms, so feeling well does not mean your blood pressure is controlled.

Monitor your blood pressure at home. A home blood pressure monitor lets you track your readings between doctor visits. Keep a log and bring it to your appointments.

Report side effects. If a side effect is bothering you, tell your doctor. There are many options within each class, and switching to a different medication or adjusting the dose can often resolve the problem.

Give it time. Finding the right medication or combination sometimes takes a few tries. This is normal and does not mean your blood pressure cannot be controlled — it means your doctor is fine-tuning the approach for your specific situation.
